Forms The forms and approvals to complete your application can be found by using the Rules Wizard. The forms are for ISEF Finalists, but we also use them for registering at ScienceMONTGOMERY. ScienceMONTGOMERY has four special rules that supplement the ISEF rules: 1. A signed Informed Parental Consent form is required for every Human Subject under 18 years of age 2. ScienceMONTGOMERY uses Categories that are different than ISEF and our Team Projects are judged in the same categories as our Individual projects. You can see all our categories on the ScienceMONTGOMERY Overview. 3. Your one-page Abstract may be submitted in your own format; the Abstract Form in the Rules Wizard is a guide, not a requirement for ScienceMONTGOMERY. (The ISEF signature page is not required and your Abstract will not be “Embossed” as “Official” by ScienceMONTGOMERY) Post your Abstract on your display board. We suggest you also leave ten copies on the table in front of your display for Judges’ use. 4. Form 1C Regulated Research Institutional/Industrial Setting must be submitted if appropriate, but must NOT be displayed with your Project All Forms -Required and Project Specific. We suggest you use the Rules Wizard, but if you need a specific form you can find it on the ISEF forms page. Required Forms for All Projects - Regardless of the type of project, all project registrations must include the following forms. Registration Form - printed from web page after completion of online registration for ScienceMONTGOMERY.  For team projects, only one member of the team should register his/her project online.  Once you have registered your project (including the names of all your team members) you will print off registration forms for everyone on your team. Checklist for Adult Sponsor/Safety Assessment Form (1) Student Checklist and Research Plan (1A), for both individuals and teams.  Includes Research Plan Attachment Approval Form 1B Each team member must submit an individual signed 1B Form. Team members must jointly submit the Checklist for Adult Sponsor (Form 1), one abstract, a Team Research Plan (Form 1A), all the Approval Forms (1B), all the Registration Forms, and other required forms in a single combined package. Be sure to read the rules for teams, which are different from the rules for individual projects.
